Assessment of Diabetes-Related Complications and Its Association with Treatment Adherence among Type 2 Diabetes Patients in Rural Banda: A Community-Based Cross-Sectional Study

Abstract
Background: Diabetes mellitus is a major public health problem, particularly in rural India where delayed diagnosis and poor treatment adherence contribute to complications. Quantifying complication burden using standardized scoring systems and understanding its association with adherence is essential. Objectives: To estimate the prevalence of diabetes-related complications among known diabetic patients using the Diabetes Complication Severity Score (DCSS) and to determine the association between treatment adherence and diabetes-related complications. Methods: A community-based cross-sectional study was conducted among 380 known type 2 diabetes mellitus patients in rural Banda district. Data were collected using a pre-tested questionnaire. Diabetes complications were assessed using a structured checklist to compute a complication severity score. Treatment adherence was measured using the Morisky Medication Adherence Scale (MMAS-8). Statistical analysis included descriptive statistics and chi-square test. Results: The prevalence of diabetes-related complications was 79.5%. A significant proportion of patients had multiple complications. Low adherence was observed in 56.3% of participants. A statistically significant association was found between poor adherence and higher complication burden (p < 0.05). Conclusion: Poor treatment adherence is significantly associated with increased complication severity. Strengthening adherence strategies can reduce diabetes-related morbidity.
